Abstract [sv]

Bakgrund: Svensk hälso- och sjukvård präglas av höga krav på effektivitet, vilket ofta krockar med sjuksköterskans professionella ansvar för en personcentrerad vård (PCV). Relationsbaserat ledarskap (RBL) har lyfts fram som en möjliggörande faktor, men kunskapen om hur sjuksköterskor erfar detta samspel inom slutenvården är begränsad. Syfte: Att utforska sjuksköterskors erfarenheter av hur relationsbaserat ledarskap inom slutenvården påverkar möjligheterna att arbeta personcentrerat. Metod: En systematisk litteraturöversikt baserad på åtta kvalitativa primärstudier. Data analyserades genom tematisk syntes och tillförlitligheten bedömdes med GRADE-CERQual. Resultat: Analysen resulterade i två huvudteman: Produktion framför omvårdnad och Ledarskapet som skyddande barriär. Resultatet visar att ett ensidigt fokus på mätbarhet och logistik förskjuter sjuksköterskans roll mot administration, medan ett närvarande och stödjande ledarskap skapar det moraliska utrymme som krävs för att bedriva PCV. Slutsats: Ett relationsbaserat ledarskap är en förutsättning för att upprätthålla omvårdnadens kärnvärden i en målstyrd organisation. Första linjens chefer behöver ges organisatoriska mandat att prioritera relationellt stöd framför administrativa krav för att säkerställa en personcentrerad vård.

Abstract [en]

Background: Swedish healthcare is currently characterized by high demands for efficiency, which often conflicts with the nursing profession’s responsibility for person-centered care (PCC). Relational leadership (RL) has been identified as a key enabling factor, yet knowledge remains limited regarding how nurses experience this interaction within inpatient settings. Aim: To explore nurses' experiences of how relational leadership in inpatient care influences the opportunities to work in a person-centered manner. Method: A systematic literature review based on eight qualitative primary studies. Data were analyzed using thematic synthesis, and confidence in the findings was assessed using GRADE-CERQual. Results: The analysis yielded two main themes: Production before nursing and Leadership as a protective barrier. The findings indicate that a one-sided focus on measurability and coordination shifts the nurse’s role toward administrative coordination. Conversely, a present and supportive leadership creates the moral space necessary for practicing PCC. Conclusion: Relational leadership is a prerequisite for sustaining core nursing values within a goal-oriented organization. To ensure person-centered care, frontline managers must be granted the organizational mandate to prioritize relational support over administrative requirements.
