Panoramic Radiographic Evaluation of Third Molar Eruption in Adolescents with Crowded Teeth: A Multicentre Randomized Controlled Trial with Fixed Appliances without Extractions

Abstract [en]

Objectives: Evaluate maxillary and mandibular third molar angulation changes before and after non-extraction treatment with fixed appliance in adolescents with dental crowding. Additionally, to evaluate the angulation depending on two different fixed appliances and the patient’s sex.

Methods: Adolescents in an RCT with fixed appliances were examined with panoramic radiography before and after treatment. The total material consisted of 132 patients. Following exclusion, 108 patients remained to be analyzed. Two independent raters measured the angulation of all maxillary and mandibular third molars. Reliability was expressed as intra- and interrater intraclass correlation coefficient with good to excellent results. Paired and independent t-tests were conducted.

Results: Maxillary third molars typically exhibited a more vertical position at T0 compared to mandibular, and paired t-tests showed no significant angulation change. Mandibular third molars had a greater inclination at T0 with paired t-tests showing a statistically significant uprighting  change in angulation following treatment (mean tooth 38 -3.1° p=0.011, tooth 48 -5.3° p<0.001). Independent t-tests showed no statistically significant differences in third molar angulation between treatment modalities or between male and female patients (p > 0.05).

Conclusions: No significant maxillary third molar angulation change was observed, reflecting stability in their alignment. Significant uprighting angulation changes in mandibular third molars were observed. This suggests that non-extraction orthodontic treatment of adolescents with dental crowding does not worsen third molar angulation. Furthermore, no significant differences in third molar angulation change were found between Damon and Victory systems or between male and female patients.

Syfte: Utvärdera vinkelförändringar av maxillära och mandibulära tredje molarer före och efter icke-extraktionsbehandling med fast apparaturhos ungdomar med trångställda tänder. Vidare att undersöka om vinkel påverkas av behandling med två olika typer av fast ortodontisk apparatur eller patientens kön.

Metod: Ungdomar i en RCT-studie med fast apparatur undersöktes med panorama-röntgenbilder före och efter behandling. Det totala studiematerialet bestod av 132 patienter. Efter exklusion kvarstod 108 patienter. Två oberoende observatörer mätte vinkling av samtliga tredje molarer i mandibeln och maxillan. Reliabiliteten uttrycktes som intra- och inter-intraklass korrelationskoefficient med goda till utmärkta resultat. Parat och oberoende t-test utfördes.

Resultat: Maxillans tredje molarer uppvisade generellt en mer vertikal position vid T0 jämfört med mandibelns och parade t-tester visade ingen signifikant förändring i vinkel. Mandibelns tredje molarer hade en större lutning vid T0, och parade t-tester visade en statistiskt signifikant upprätande förändring i vinkel efter behandling (medel tand 38 -3.1° p=0.011, tand 48 -5.3° p<0.001). Oberoende t-tester visade inga statistiskt signifikanta skillnader i vinkelförändring av visdomständerna mellan behandlingsmodaliteter  eller mellan män och kvinnor (p > 0,05).

Slutsats: Ingen signifikant förändring i vinkeln av maxillära tredje molarer observerades vilket indikerade stabilitet i deras vinkel. Signifikanta upprätande förändringar i vinkeln av mandibelns tredje molarer observerades. Detta tyder på att ortodontisk behandling utan extraktioner hos ungdomar med trångställda tänder inte förvärrar vinkeln av tredje molarer. Dessutom observerades inga signifikanta skillnader i vinkeln av tredje molarer vid jämförelse av Damon- och Victory-systemen eller män och kvinnor.
