Syfte: Denna systematiska översikt syftar till att kartlägga publicerad evidens om placeboeffekten vid diagnostik och/eller behandling av akut orofacial smärta.

Material och metod: I november 2020 utfördes en systematisk litteratursökning i PubMed, Cochrane och Scopus. Kliniska studier som behandlade användningen av placebo vid akut orofacial smärta och som hade publicerats år 2010 eller senare inkluderades i detta arbete. Den systematiska översikten gjordes enligt PRISMA-riktlinjerna och Cochranes manual för systematiska översikter och har följt ett protokoll registrerat i PROSPERO.

Resultat: Databassökningen resulterade i totalt 1477 artiklar. Efter titel- och abstraktgranskning fulltextgranskades 450 artiklar och 78 artiklar uppfyllde inklusionskriterierna. En handsökning identifierade ytterligare 120 artiklar som uppfyllde inklusionskriterierna vilket resulterade i totalt 198 relevanta artiklar för kliniska studier om akut orofacial smärta. Efter avgränsning till åren 2010–2020 inkluderades 40 artiklar. En studie undersökte diagnostik och 39 behandling. Av de 40 studierna visade 31 att aktiva interventioner hade bättre effekt än placebon. Ingen av studierna visade att placebo hade bättre effekt än en aktiv intervention.

Slutsats: Omfattningen av placeboeffekten vid behandling och diagnostik av akut orofacial smärta kunde inte fastställas i denna systematiska översikt. I en majoritet av studierna sågs en större effekt av den aktiva behandlingen jämfört med placebobehandlingen.

Aim: The aim of this systematic review was to identify published clinical studies using placebo for medical diagnosis and/or treatment in individuals with acute orofacial pain in order to study the placebo effect.

Materials and Methods: A systematic search was conducted in November 2020 using PubMed, Cochrane and Scopus. Clinical studies investigating placebo and acute orofacial pain, published 2010 or later, were included in this project. The systematic review was conducted according to the PRISMA-guidelines and Cochranes manual for systematic reviews and followed a protocol registered in PROSPERO. 

Result: The electronic data search resulted in a total of 1477 articles. After title and abstract screening a total of 450 were reviewed in full text and 78 articles met the inclusion criteria. During the hand search an additional 120 articles that met the inclusion criteria was identified which resulted in a total of 198 published clinical studies regarding acute orofacial pain. After restricting the present report to the publication years 2010–2020, 40 articles were included. One study investigated diagnostics and 39 treatments. A total of 31 out of the 40 studies showed that the active intervention had better effects than the placebo. None of the studies found a placebo intervention more effective than an active intervention.

Conclusion: The magnitude of placebo effect in diagnostics and treatment of acute orofacial pain could not be estimated in this systematic review. A greater effect of the active treatment could be seen in a majority of the studies compared to the placebo treatment.
