I den dagliga kontakten med patienter med endotrakealtub och trakealkanyl som respiratorbehandlas noterades tendens till oförväntade tecken på luftvägsstopp. Syftet var att genom en litteraturstudie med systematisk metodansats identifiera beskrivna sugtekniker för att säkerställa fria luftvägar på patienter med endotrakealtub/trakealkanyl. Resultatet visar en fördel med att använda böjd sugkateterspets för att nå en specifik lunga och att slutet sugsystem inte är mer effektivt än öppet avseende mängden uppsuget sekret samt antal sugtillfällen. Ett visst mått av rutinmässig sekretsugning per dag är till godo för patienten trots avsaknad av sekret. Att arbeta utifrån nationella riktlinjer är säkert bra men det är också nödvändigt att respektive klinik med dess kategori av patienter förhåller sig kritisk och vid behov utfärdar lokala föreskrifter och rutiner.

In the daily care of patients with endotracheal tube and tracheal cannula who receive mechanical ventilation, unexpected signs of airway occlusion were noticed.The aim of this literature review with an intention to systematic method was to indentify described suction techniques to secure free airway. The findings show a benefit using a curved suction catheter tip to reach a specific lung and that a closed suction system is not more effective than an open system considering amount of secretion removed and number of suction passes. Further the findings show that despite the lack of secret a matter of routine suctioning is important and will be good for the patient. It is of great value to follow national guidelines but it is necessary and up to each and every clinic, according to their specific category of patients, to make their own judgement if local directions and routines have to be made.
