Fracture Strength and Fracture Mode of Monolithic Multi-layered Zirconia Fixed Dental Prostheses with Different Nesting Strategies – An in vitro study

Abstract [sv]

Syfte: Syftet med denna studie är att undersöka hur effekten av CAD-strategin för placering av treledsbroar i de flerskiktade zirkoniablocken påverkar brotthållfastheten samt frakturmönstret hos broarna. Material och metod: Trettio treledsbroar framställdes och delades in i tre grupper (n=10) beroende på de olika positionerna av CAD-figurerna i de flerskiktade zirkoniablocken. Broarna delades in enligt följande: broar med connectorerna placerade i det translucenta lagret (T), broar med connectorerna placerade i mellanlagret (G) och broar med connectorerna placerade i dentinlagret (D). Kontrollgruppen (n=10) utgjordes av monolitisk translucent zirkonia fräst ur ett enskiktat block (C). Samtliga broar utsattes för termocykling, cyklisk förbelastning och belastades slutligen till brott. Brotthållfasthet samt frakturmönster analyserades statistiskt (p ≤0.05). Resultat: Det fanns ingen statistisk signifikant skillnad avseende brotthållfasthet bland grupperna baserat på CAD-strategin för placering av treledsbroar i det flerskiktade zirkoniablocket (p >0.05). Frakturmönstret hos broarna visade dock en statistisk signifikant skillnad mellan grupperna C/D och T (p= 0.007). Slutsats: CAD-strategin för placering av treledsbroar i de flerskiktade zirkoniablocken har ingen betydande effekt på brotthållfastheten hos broarna. Flerskiktad zirkonia verkar ha en likvärdig hållfasthet jämfört med monolitisk enskiktad translucent zirkonia, men frakturmönstret kan skilja sig åt.

Abstract [en]

Purpose: The aim of this study was to evaluate the effect of nesting, (in this study the CAD strategy for placement of fixed dental prostheses (FDPs) in multi-layered translucent zirconia blocks) on fracture resistance and fracture mode of monolithic FDPs. Material and methods: Thirty three-unit zirconia FDPs were fabricated and divided into three groups (n=10) according to the different positions of the CAD shapes in multi-layered zirconia blocks. The FDPs were divided as follows: FDPs with the connectors placed in the translucent layer (T), FDPs with the connectors placed in the gradient layer (G) and FDPs with the connectors placed in the dentin layer (D). A final group (n=10) of monolithic translucent zirconia milled from a single layered block was used as control (C). The specimens were artificially aged using thermocycling and pre-loading procedures and subsequently loaded to fracture. Fracture loads and fracture modes were analyzed and statistically evaluated (p ≤0.05). Results: There were no significant differences in fracture load among the groups based on the nesting of the connector area of the FDPs in the multi-layered zirconia block (p >0.05). Fracture mode of the FDPs showed, however, significant differences between group C/D and T (p= 0.007). Conclusion: The nesting strategies do not have a considerable effect on the fracture resistance of FDPs made of multi-layered zirconia. The multi-layered zirconia seem to have comparable strength to monolithic single-layered translucent zirconia, but fracture mode might differ.
