Does patient age affect marginal peri-implant bone levels?: A retrospective radiographic study with at least 3 years follow-up.

Abstract [en]

Aim: To evaluate the effect of age on marginal peri-implant bone loss (MPI-BL).

Materials and Methods: Patient records were screened retrospectively for the following inclusion criteria: Radiograph (1) at implant installation, and (2) after ≥3 years. Two-hundred-sixty-four patients/implants were included with a mean follow-up of 5.8 years (3.1-13.1 years). Three different age groups were matched to each other, 35 to <55, 55 to <65, and ≥65 years. Associations between patient age as main predictor and other patient-, implant- and prosthetic-related parameters were assessed with radiographic MPI-BL based on median regression models.

Results: The average MPI-BL was 1.3 mm (±1.3), 1.4 mm (±1.2), and 1.3 mm (±1.0) in the young, middle-aged, and old group, while the maximum MPI-BL was 1.7 mm (±1.3), 1.7 mm (±1.3), and 1.6 mm (±1.3). MPI-BL <2 mm, 2 to 5 mm, and ≥5 mm occurred in 65.9%, 32.2% and 1.9% of the implants, but not a single implant in the old group presented with ≥5 mm MPI-BL. Age did not show any significant effect on MPI-BL. 

Conclusions: MPI-BL ≥2 mm occurred in approximately one third of the cases, however, the patients age has no impact. This suggests that elderly do not necessarily have an increased risk for peri-implant bone loss.

Syfte: Att utvärdera effekten av ålder på marginal peri-implantär benförlust (MPI-BL).

Material och metod: Patientjournaler screenades retrospektivt för följande inklusions kriterier: Röntgenbild (1) vid implantatinstallation, och (2) efter ≥3 år. Tvåhundrasextiofyra patienter/implantat inkluderades med en genomsnittlig uppföljning på 5,8 år (3,1–13,1 år). Tre olika åldersgrupper matchades till varandra, 35 till <55, 55 till <65 och ≥65 år. Samband mellan patientens ålder och andra patient-, implantat- och protetikrelaterade parametrar bedömdes med röntgenologisk MPI-BL baserat på̊ medianregressionsmodeller.

Resultat: Den genomsnittliga MPI-BL var 1,3 mm (±1,3), 1,4 mm (±1,2) och 1,3 mm (±1,0) i den unga, medelålders och äldre gruppen, och den maximala MPI-BL var 1,7 mm (±1,3), 1,7 mm (±1,3) och 1,6 mm (±1,3). MPI-BL <2 mm, 2 till 5 mm och ≥5 mm förekom hos 65,9%, 32,2% respektive 1,9% av implantaten men inte ett enda implantat i den äldre gruppen visade en benförlust ≥5 mm. Åldern har inte någon signifikant påverkan på MPI-BL. 

Konklusion: MPI-BL ≥2 mm inträffade vid ungefär en tredjedel av implantaten, och patientens ålder påverkar ej. Detta tyder på att äldre inte nödvändigtvis har en ökad risk för marginal peri-implantär benförlust.
