Abstract [sv]

Vänsterkammares ejektionsfraktion (VKEF) är idag en av de vanligaste och mest betydelsefulla parametrarna vid ekokardiografiska under-sökningar. VKEF ger ett mått på hur stor mängd slutdiastolisk kammarvolym som pumpas ut under systole. Idag är tvådimensionell ekokardiografi (2DE) den mest förekommande metoden för undersökning av VKEF där andra metoder kan implementeras så som mitral annular plane systolic excursion (MAPSE) och speckle tracking. Studien undersöker sambandet mellan metoderna MAPSE, speckle tracking samt tredimensionell ekokardiografi (3DE) avseende bedömningen av longitudinell väggrörlighet i vänster kammare samt VKEF. Longitudinell väggrörlighet i vänster kammare samt VKEF mättes med metoderna 2D-speckle tracking, MAPSE och 3DE på 25 patienter med olika frågeställningar remitterade för en ekokardiografisk undersökning. Pearsons rangkorrelationstest visar på ett signifikant samband mellan metoderna MAPSE och speckle tracking avseende bedömningen longitudinell väggrörlighet (r= 0,415). Signifikant resultat erhölls även för metoderna 3DE och 2DST avseende bedömning av VKEF genom en Bland-Altman analys (r=0,498). Ett signifikant samband mellan MAPSE och 2DST avseende be-dömningen av longitudinell väggrörlighet i vänster kammare finns. Avseende be-dömningen av VKEF erhålls resultat som tyder på signifikant skillnad i bedömningen.

Abstract [en]

Left ventricular ejection fraction (LVEF) is one of the most com-mon and most significant parameters in today’s echocardiographic exams. LVEF is a measurement of the amount of end-diastolic ventricular volume which is ejected out during systole. Although, two-dimensional echocardiography (2DE) is the most common method to examine LVEF, other methods can be implemented with 2DE such as mitral annular plane systolic excursion (MAPSE) and speckle tracking. The study examines the relationship between the methods MAPSE, speckle tracking, and three-dimensional echocardiography (3DE) regarding the assessment of longitudinal wall movement in left ventricular and LVEF. Longitudinal wall movement in the left ventricular and LVEF was measured with the methods, 2D- speckle tracking, MAPSE and 3DE on 25 patients with different issues, which were referred for an echocardiographic exam. Results: Pearson’s rank correlations test indicates a significant correlation between the methods MAPSE and speckle tracking regarding the assessment longitudinal wall movement (r=0.415). Significant results were also obtained for the methods 3DE and 2DST regarding the assessment of LVEF with a Bland-Altman analysis (r=0.498). A significant correlation exists between MAPSE and 2DST regarding the assessment of longitudinal wall movement of the left ventricle. Regarding the assessment of LVEF, obtained results, indicate a significant difference.
