Abstract [sv]

Bakgrund: Barnfetma rapporteras vara ett av de största världsomfattande folkhälsoproblem som finns i nutid. Skolsköterskan med uppdrag att arbeta hälsofrämjande träffar samtliga elever på en skola med ansvar att sätta in åtgärder till barn med ohälsosam vikt. Tidigare studier påvisar att skolsköterskor anser övervikt och fetma hos barn vara komplext och svårt att arbeta med. Syfte: Syftet med studien var att belysa skolsköterskors upplevelse av kommunikation med barn och vårdnadshavare om barnets övervikt eller fetma. Metod: Kvalitativ metod med semistrukturerade intervjuer användes, elva skolsköterskor deltog. Materialet analyserade med tematisk innehållsanalys. Resultat: Resultatet redovisas i fyra kategorier med tillhörande underkategorier. De fyra kategorierna är: Strategier, Varierande reaktioner, Förhållningssätt och Flertalet utmaningar. Av resultatet framkom att skolsköterskorna sällan kommunicerade enskilt med barnen gällande deras övervikt eller fetma, istället påpekades vårdnadshavarnas betydelsefulla roll. Variationer förelåg i hur skolsköterskorna upplevde att barnen och/eller vårdnadshavarna reagerade i samtalen. Emellertid påpekade skolsköterskorna att ämnet avseende övervikt och fetma hos barn var komplext där skolsköterskorna vidtog försiktighet i samtalen. Slutsats: Studiens resultat lyfter fram skolsköterskornas upplevelser av kommunikation med barn och vårdnadshavare om barnets övervikt eller fetma. I resultatet skildras strategier i hur samtalen strukturerades men också förhållningssätt som upplevdes mer eller mindre gynnsamma.

Abstract [en]

Background: Child obesity is reported to be one of the largest worldwide public health problems that exist today. The school nurse with the task of promoting health promotion meets all the students at a school with the responsibility of introducing measures to children with unhealthy weight. Previous studies have shown that school nurses consider overweight and obesity in children to be complex and difficult to work with. Aim: The aim of this study was to illuminate school nurses experience of communication with children and caregivers about the child’s overweight or obesity. Method: Qualitative method with semi-structured interviews was used, eleven school nurses participated. The material was analyzed using thematic content analysis. Result: The result is reported in four categories with associated subcategories. The four categories are: Strategies, Different reactions, Attitudes and Challenges. The results showed that the school nurses rarely communicated individually with the children regarding their overweight or obesity, instead the important role of the guardians was pointed out. There were variations in how the school nurses felt that the children and/or guardians reacted in the conversations. However, the school nurses pointed out that the topic of overweight and obesity in children was complex where the school nurses took precaution in the conversations. Conclusion: The study results highlight the school nurses´ experiences of communication with children and caregivers about the child’s overweight or obesity. The result describes strategies in how the conversations were structured but also approaches that could be favorable.
