Abstract [sv]

Bakgrund: Enligt socialstyrelsen insjuknar 40 av 100 000 personer i Sverige varje år i typ 1-diabetes. Minst 30 % av alla patienter som behandlas med insulin drabbas någon av allvarlig hypoglykemi. Svettningar, hög hjärtfrekvens med hjärtklappning, hungerskänslor, ångest och svaghet är klassiska symtom som obehandlat kan övergå i medvetslöshet, koma och död. Då typ 1-diabetes upplevs olika från patient till patient krävs en personcentrerad omvårdnad av sjuksköterskan. Syfte: Syftet är att sammanställa forskning för att undersöka erfarenheter av allvarlig hypoglykemi, samt egenvård och omvårdnad kring tillståndet hos patienter med typ 1-diabetes Metod: Litteraturstudie med innehållsanalys av 14 studier med kvalitativ ansats. Resultat: Analysering av studierna resulterade i fyra tema: erfarenhet av allvarlig hypoglykemi, rädsla för allvarlig hypoglykemi, egenvård och slutligen omvårdnad. Konklusion: Patienternas tidigare erfarenheter av allvarlig hypoglykemi medför förändrade egenvårdsåtgärder samt ställer utökade krav på omvårdnad. Rädsla för allvarlig hypoglykemi är förenat med kortsiktiga proaktiva metoder i undvikande syfte. Patientens roll som partner i omvårdnaden, samt sjuksköterskans roll som utbildare och emotionellt stöd skildras som välbehövlig. Sjuksköterskans förmåga att bjuda in patienten i omvårdnadsarbetet och gemensamt utforma en personcentrerad egenvård är essentiellt. Abstract [en]

Background: According to socialstyrelsen 40 of 100 000 people in Sweden become ill with type 1-diabetes every year. At least 30 % of every patients that gets treated with insulin suffers occasionally from severe hypoglycemia. Sweating, high heart rate with palpitations, feelings of hunger, anxiety and weakness are classic symptoms that if left untreated results in unconsciousness, coma and death. As diabetes type 1 is perceived differently from patient to patient the nurses’ ability to perform person centred care is thereby essential. Aim: The aim is to compile research to investigate the experience of severe hypoglycaemia, as well as self-care and care about the condition of patients with type 1 diabetes.. Method: A literature study that content analysis of 14 studies with qualitative approach. Results: Analysis of the studies resulted in four themes: experience of severe hypoglycaemia, fear of severe hypoglycemia, self-care and nursing. All the participants påpekar det oumbärliga med personcentrerad omvårdnad. Conclusion: The patient's previous experience of severe hypoglycaemia leads to altered self-care measures and increases the requirements for nursing care. Konklusion: The patients past experiences with severe hypoglycemia entails changed measures of self care together with higher demands in nursing care. Fear of severe hypoglycemia is associated with short term proactive methods of avoiding severe hypoglycemia. The patients role as an associate in nursing together with the nurse role as educator and emotional support is presented as vital. The nurse ability to invite the patient as a member of the nursingprocess as well as the ability to design person centred care is essential.
