Abstract [sv]

Syftet med denna litteraturstudie var att sammanställa vetenskaplig kunskap om vilka rapporteringssystem som finns samt om något av dem leder till större patientsäkerhet. Metoden som har använts för att få fram resultatet är en systematisk litteraturstudie där vetenskapliga artiklar granskas. Det finns fyra olika rapporteringssätt: muntlig rapport, bandspelarrapport, tyst rapport och bedsiderapport. Inget av dem leder ensamt till ökad patientsäkerhet. Resultatet baseras på nio vetenskapliga artiklar. Resultatet visar att då muntlig rapport används kan informationen som tas upp vara irrelevant för patientvården. Det kan bli sjuksköterskans egna personliga värderingar om patienter som tas upp och viktig information faller i glömska, vilket kan spela en avgörande roll för kvaliteten på vården och patientsäkerheten. Ytterligare forskning kring rapporteringssystem och patientsäkerhet behövs och då mer inriktad på empiriska studier.

Abstract [en]

The aim of this literature review was to compile scientific knowledge of which systems of reporting that exists and if any of them will lead to a greater patient safety. The methods used to bring out the results is a systematic literature review were scientific articles has been examined. There are four ways to carry out a report: the verbal handover, the taped report, the silent/written report and the bedside report. Neither of those will alone lead to an increased safety for the patients. The result is based on nine scientific articles. The result indicates that when verbal handovers are used the information can be irrelevant to the patient care. The nurses’ personal judgements about the patients are often brought up during the verbal handover and important information is forgotten. This can play a vital role for the care of the patients and also their safety. Further research about systems of reporting and patient safety is needed and then in an empirical style.
