Abstract [sv]

Syftet var att sammanställa vetenskaplig litteratur med avsikt att undersöka betydelsen av regelbundet byte alternativt byte vid klinisk indikation av perifer venkateter (PVK) för att minska frekvensen och allvarlighetsgraden av tromboflebit hos vuxna patienter under sjukhusvistelse. Litteraturstudien utfördes systematiskt i sju steg enligt SBU (1993). Totalt inkluderades 10 studier efter databassökningar i PubMed, Cinahl och Cochrane Library. Studierna granskades av två oberoende bedömare utifrån ett modifierat granskningsprotokoll enligt Willman et al (2006) och evidensgraderades därefter. Resultatet visade motstridiga evidens avseende regelbundet byte alternativt byte vid klinisk indikation gällande minskning av frekvens och allvarlighetsgrad av tromboflebit. Fler RCT borde utföras för att med starkare evidens kunna rekommendera vilken av interventionerna som är lämpligast och dessa borde undersöka allvarlighetsgraden. Framtida forskning borde även eftersträva en standardiserad definition av tromboflebit.

Abstract [en]

The aim was to compile scientific literature to investigate the effect of routine replacement versus clinically indicated resite of peripheral venous catheter (PVC) to reduce the incidence and severity of thrombophlebitis in hospitalized adults. The literature review followed a systematic approach in seven steps according to SBU (1993). A total of 10 studies were included after searches in the databases PubMed, Cinahl and Cochrane Library. The studies were scrutinized with a modified protocol from Willman et al (2006) by two independent readers and were then judged by quality. The result indicated conflicting evidence according to routine replacement and clinically indicated resite to reduce the incidence and severity of thrombophlebitis. More clinical randomized trials with stronger evidence are necessary to be able to recommend which of the interventions that is most appropriate. A standardized definition of thrombophlebitis would be desirable in future research.
