Abstract [sv]

Testosteron är ett könshormon som finns hos både män och kvinnor. Detta hormon finns till största del i bunden form i blodet, då bundet med hög affinitet till sexualhormonbindande globulin (SHBG) eller med låg affinitet till albumin. Den fria formen av testosteron utgör endast 0-2 % och svarar för den biologiska aktiviteten. Serum genomgick ultrafiltrering eller rapid equilibrium dialysis (RED) vid antingen rumstemperatur eller 37 °C därefter kvantifierades fritt testosteron med LC-MS/MS. Efter utvärdering genomfördes en jämförelse mot den nuvarande analysmetoden, total testosteron/SHBG-kvot. Slutligen undersöktes medelvolym av filtrat, albuminförekomst i filtrat samt inbindning till provbehållare. Resultaten från jämförelse av RED och ultrafiltrering vid rumstemperatur och 37 °C gav p-värden på, 0,892 och 0,696 respektive. Därmed ses ingen signifikant skillnad mellan provberedning med RED jämfört med ultrafiltrering. Vid jämförelse mellan ultrafiltrering och total testosteron/SHBG-kvot av patientprov gavs R-värde på 0,9089 och 0,879 vilket indikerar ett bra linjärt samband. Dock krävs vidare jämförelser med laboratorier som koncentrationsbestämmer fritt testosteron i serum med RED för att bekräfta överenstämmelsen mellan ultrafiltrering och RED innan analysen kan sättas i bruk. Resultaten från albuminförekomst i filtrat visade att albumin inte släpps igenom filtren om de inte punkterats med ett spetsigt föremål, då sågs albuminkoncentrationer likt de i serum. Albumin anses därför vara en lämplig kontroll av ultrafiltrering.

Abstract [en]

Testosterone is a sex hormone found in both men and women. This hormone is predominantly found in its bound form in blood, either bound with high affinity to sex hormone binding globulin (SHBG) or with low affinity to albumin. The free form of testosterone represents only 0-2 % of the total amount of testosterone but stands for the biological activity. Serum underwent ultrafiltration or rapid equilibrium dialysis (RED) in either room temperature or 37 °C and was then analyzed with LC-MS/MS. After evaluation a comparison with the current method, total testosterone/SHBG-index, was made. Lastly the mean filtrate volume, presents of albumin in filtrate and binding of free testosterone to sample chamber were studied. The results from the comparison of RED and ultrafiltration at room temperature and 37 °C gave p-values of 0,892 and 0,696. Therefore, no significant difference between sample preparation with RED or ultrafiltration could be seen. The comparison of ultrafiltration and total testosterone/SHBG-ratio of patient samples resulted in R-values of 0,9089 and 0,879, which indicates a good linearity between the methods. However, further comparisons with laboratories who determine the free testosterone concentration in serum with RED is needed to confirm the agreement between ultrafiltration and RED before the analysis is put into operation. The results obtained from examining the presence of albumin in filtrate showed that no albumin was let through the filter unless the filter was punctured with a sharp object, then the albumin concentration equaled the amount found in serum. Albumin is therefore considered an appropriate control for ultrafiltration.
