Abstract [sv]

Hypertoni innebär förhöjt blodtryck. Det utgör en riskfaktor för utveckling av hjärt-kärlsjukdomar och njursjukdomar. Därför det är viktigt att diagnoserna hy-pertoni genom blodtrycksmätning. Blodtryck kan mätas båda automatisk och ma-nuellt. Vid ambulatorisk blodtrycksmätning under 24-timmar används automatisk mätmetod. Vid varje undersökning kontrolleras utrustning med hjälp av manuell blodtrycksmätning. Detta innebär att man samtidigt mäter blodtrycket och avläser värdet på en display på den ambulatoriska mätaren och på manometer på den ma-nuella. Detta upprepas tre gånger och genomsnittsskillnaden beräknas för respek-tive metod. Enligt metodbeskrivning vid Skånes universitetssjukhus i Lund ska medelskillnaden ligga under 9 mmHg och enligt European Society of Hypertens-ion (ESH) och European Society of Cardiology (ESC) guidlines ska skillnaden vara ≤5 mmHg i minst 60 % av mätningarna. Syftet med denna retrospektiva stu-die som innefattade 100 patientmätningar var att undersöka hur ofta medelvärdet mellan automatisk och manuell metod låg över 5 mmHg samt hur stor skillnaden var. Resultatet visar att i 16 % av de systoliska patientmätningar och i 10 % av de diastoliska mätningarna var skillnaden större än 5 mmHg. Den största skillnaden vid systolisk tryckmätning var 10 mmHg och vid diastoslik tryckmätning 8 mmHg. Studien visar att skillnaden mellan mätmetoderna är små vilket innebär att ESH och ESC riktlinjer uppfylls.

Abstract [en]

Hypertension, which is the medical term for high blood pressure, is considered a risk factor for coronary artery disease and kidney disease. This means that the di-agnosis of hypertension, which is done by the measurement of blood pressure, is very important. Blood pressure can be measured either by an automated or a man-ual method. When an ambulatory blood pressure monitoring during 24 hours is performed, blood pressure is measured through automated equipment. The equip-ment is controlled through the means of a manual blood pressure measurement. This is done by simultaneously measuring a patient’s blood pressure manually and comparing the pressure given by the automated display with the pressure shown on the manometer. This is repeated three times and the mean difference is calcu-lated for each method. According to the guidelines used in Skåne University Hos-pital in Lund, the mean difference cannot be greater than 9 mmHg. The European Society of Hypertension (ESH) and the European Society of Cardiology (ESC) guidelines state that mean difference is required to be under 5 mmHg. This study was a retrospective study which included 100 patient measurements. The purpose of this study was to evaluate how frequently the mean difference between the automated method and the manual method surpassed 5 mmHg in least 60 % of measurements. The results show that 16 % of the systolic measurements and 10 % of the diastolic measurements had a mean difference surpassing 5 mmHg. The largest difference for the measurement of systolic pressure was 10 mmHg while the largest difference for diastolic measurements was 8 mmHg. The study indi-cates that the difference between the two methods is very small, meaning that the ESH and ESC guideline requirements are met.
