Abstract [sv]

Vårt syfte med denna undersökning är att undersöka hur övergången från barnhabiliteringen till vuxenhabiliteringen fungerar. Vårt syfte är också att undersöka de likheter och skillnader som finns mellan barnhabiliteringen och vuxenhabiliteringen samt vad dessa kan ha för betydelse vid en övergång. Vidare har vi undersökt vilka problem som kan uppstå vid en övergång. Uppsatsgruppen har undersökt Region Skånes habiliteringar, där sex intervjuer har gjorts med de professionella. Vi har intervjuat tre professionella inom barnhabiliteringen och tre inom vuxenhabiliteringen. Undersökningen lyfter fram tre teoretiska perspektiv; Erikssons teori, Empowerment och ett salutogent perspektiv. Uppsatsgruppen vill påpeka att det är de professionellas olika synvinklar som undersöks efter syftet och inte individen själv som annars tillhör barnhabiliteringen eller vuxenhabiliteringen.

Abstract [en]

Our purpose of this study is to examine the transition from childhabilitation to adulthabilitation. It also aims to investigate childhabilitation and adulthabilitation similarities and differences and what they may have on role in a transition. Furthermore, we examine the problems that can arise in a transition from childhabilitation to adulthabilitation. Our study comprises a total of six interviews with professionals in Region Skåne habilitation. We have interviewed three professional in childhabilitation and three professional in adulthabilitation. In the absence of previous research into how the transition works and what problems can arise in a transition, it was important to interview the professionals who work directly with the transition from childhabilitation to adulthabilitation. The study considers three theoretical perspective, Erikson's theory, Empowerment and the salutogenic perspective. The similarities and differences that appear in the results of the study include the design of the habilitation team, image of self-responsibility of the individual, the purpose of the habilitation plan, or any form of support and intervention. Our results on the transition from childhabilitation to an adulthabilitation shows including when a transition takes place, if everyone makes a move, how a switch works and the problems that may arise in a transition. The similarities and differences between childhabilitation and adulthabilitation demonstrates that there may be a role for a transfer. Among other things, aid and interventions designed similar between habilitation makers, have continued to run for the individual at a transition. Support and efforts have a different meaning for a transfer when half of the habilitation team that existed for the individual in childhabilitation is no longer current in adulthabilitation.
