Trycksår är en vanlig komplikation hos patienter som drabbats av höftfraktur. Syftet med denna systematiska litteraturstudie var att finna betydelsefulla riskfaktorer för trycksår hos patienter med höftfraktur för att sjuksköterskan omgående ska kunna identifiera en patient med hög risk. Metoden som använts för denna litteraturstudie var en modell med sju steg för ett systematiskt tillvägagångssätt. Litteratursökningen utfördes i databaserna PubMed, CINAHL och The Cochrane Library. Kvalitetsgranskning av studier utfördes med ett protokoll av Willman m fl (2006) modifierat av författarna. Resultatet av åtta inkluderade studier visar att flera olika faktorer verkar påverka utvecklingen av trycksår hos patienter med höftfraktur. Efter evidensgradering med hjälp av ett verktyg av Britton (2000) kunde följande riskfaktorer relateras till ett starkt vetenskapligt underlag: Hög ålder, kognitiv dysfunktion, kvinnligt kön, diabetes mellitus, låg ASA bedömning, malnutrition, immobilisering, lång väntetid till operation och hög risk enligt riskbedömningsinstrument.

Abstract [en]

Pressure ulcers are a common complication in patients suffering from a hip fracture. The aim of this systematic review was to find important riskfactors for development of pressure ulcers in patients suffering a hip fracture so that the nurse immediately can identify a patient with an increased risk. The method used for this systematic review was a model with seven steps for a systematic approach. The literature search was performed in the databases PubMed, CINAHL and the Cochrane Library. The quality review of included studies was performed with a protocol by Willman et al (2006) modified by the authors. The result of eight included studies showed that several factors seem to affect the development of pressure ulcers in patients with hip a fracture. After having evaluated the evidence with a tool by Britton (2000) the following riskfactors were related with a strong scientific basis: High age, cognitive dysfunction, female gender, diabetes mellitus, low ASA-grade, malnutrition, immobility, long waiting time before surgery and a high risk according to risk evaluation tests.
