Do you have caries? A comparison of DIAGNOcam examination to bitewing radiographs as a diagnostic method for proximal caries in primary molars - A pilot study

Abstract [sv]

Syftet med studien var att utvärdera den diagnostiska tillförlitligheten hos Digital Imaging Fiber­ Optic Transillumination (DIAGNOcam (DC)) med digital bitewingröntgen (BW), som referenstest, för att detektera approximalkaries i primära tänder. Studien undersökte även barns upplevelse av DC jämfört med BW som undersökningsmetod. Femton barn (ålder 4-8) inkluderades i studien och totalt 73 (BW) respektive 87 (DC) tänder undersöktes. Undersökningen med DC utfördes efter eller i samband med ordinarie undersökningstillfälle och BW-undersökning. Lämpliga kariesklassificeringssystem användes. Direkt efter varje undersökning (DC och BW) utvärderades barnens upplevelse med Faces Pain Scale (FPS). Data från DC jämfördes med BW som referenstest. En signifikant skillnad mellan de två metodernas förmåga att upptäcka karies (P=0,027) hittades. För DC var sensitiviteten 48 %, specificiteten 78 % och tillförlitligheten 69 % (dentinkaries), jämfört med BW som referenstest. Dentinkaries upptäcktes på dubbelt så många ytor med BW jämfört med DC. lngen statistisk analys kan göras med data från FPS på grund av det lilla studiematerialet. FPS visade att barnen graderade DC som en mer behaglig upplevelse an BW. BW visar en större variation i resultatet. Denna pilotstudie indikerar på att den diagnostiska tillförlitligheten hos DC inte är lika bra som hos BW på att upptäcka approximalkaries i primära tänder, men barnen verkar accepterar metoden bättre. Resultaten måste tolkas med försiktighet på grund av det lilla studiematerialet.

Abstract [en]

The aim of this study was to evaluate the diagnostic accuracy of Digital Imaging Fiber-Optic Transillumination (DIAGNOcam (DC)) using digital bitewing radiographs (BW) as reference test, to detect proximal caries in primary teeth. The study also investigated how children experience DC as an examination method compared with BW. Fifteen children (age 4-8) were included and a total of 73 (BW) respectively 87 (DC) teeth were examined. Examination with DC was proceeded after or in connection with ordinary dental examination and BW. Appropriate caries classification system was used. After each examination an assessment of the childrens' experience was investigated with Faces Pain Scale (FPS). The data from DC were compared with BW as reference test. A significant difference between the two method's abilities to detect caries (P=0.027) was found. For DC, sensitivity was 48 %, specificity 78 % and accuracy 69 % (dentine caries) using BW as reference test. Dentine caries was detected on twice as many sites with BW compared to DC. No statistical analysis can be made with the data from FPS because of the small size of data. According to FPS children rated DC as a more pleasant experience compared to BW. BW showed a wider variation in the result. This pilot study might indicate that DC is not as accurate as BW as a diagnostic method to detect proximal caries in primary teeth, but the children seem to accept the method better. The result needs to be interpreted carefully because of the small size of study material.
