Brotthållfasthet hos helkeramiska (Y-TZP) implantatunderstödda singelkronor, cementerade kronor jämfört med skruvade

Abstract [sv]

Syfte Syftet med studien är att undersöka hållfasthetsskillnader för yttriumstabiliserade tetragonala zirkoniumdioxid-distanser (Y-TZP) vid cementering kontra skruvretinering av zirkoniakronor. Material och metod Tjugotvå överkonstruktioner fastsatta på fixturer delades upp i två grupper. En grupp bestod av cementerade zirkoniakronor på en zirkoniadistans. Den andra gruppen bestod av skruvretinerade zirkoniakronor direkt på fixturnivå. Samtliga kronor som vanligtvis skulle genomgått porslinspåbränning fick genomgå bränningsprogram i en porslinsugn. Sedan skruvades fixturerna ned i plexiglas-block för att sedan kunna montera överkonstruktionerna. Samtliga provkroppar genomgick sedan artificiellt åldrande genom termocykling samt cyklisk förbelastning. Samtliga provkroppar utsattes slutligen för belastning till fraktur eller deformation. Resultat Ingen signifikant skillnad kunde ses mellan de två undersökta grupperna (p < 0.05). I 20 av de sammanlagt 22 provkropparna kunde en tydlig deformation av distansskruven ses. Sammanräknat mellan de båda två grupperna inträffade totalfraktur av överkonstruktionen i 8 av 22 fall. Slutsats Trots begränsningen vid en in vitro studie kunde det fastställas att den svagaste delen av överkonstruktionen var distansskruven. Det kunde inte fastställas någon hållfasthetsskillnad mellan de cementerade eller skruvretinerade zirkoniakronorna. Det krävs mer liknande studier innan man kan fastställa resultatets kliniska relevans.

Abstract [en]

Objective The aim of this study was to investigate the fracture strength and fracture mode of yttria-stabilized tetragonal zirconia polycrystal (Y-TZP) abutments, and to examine if there is a difference between a cemented crown or screw-retained crown. Methods Twenty-two single implant crowns were divided into 2 groups where one group had a cemented Y-TZP crown and the other group had a screw-retained crown. All the Y-TZP specimens which usually is veneered underwent a simulated aging process including heat treatment by a porcelain furnace. Then the fixtures were assembled with its polymer block and the crowns fitted on the abutments. All blocks with fixtures and cemented/screw-retained crowns underwent thermocycling and cyclic preload. All specimens were then subjected to load until fracture. Results No significant difference was seen between the two groups (p < 0.05). In 20 of the 22 cases the deformation occured in the abutment screw. In 8 of the 22 cases the Y-TZP superstuctures underwent a complete fracture. Conclusions Although the limitations of an in vitro study, it can be concluded that the weakest part of the superstructure was the abutment screw. There was no difference seen between the groups of cemented and screw-retained Y-TZP crowns. This result needs to be verified by further studies before considered clinically applicable.
