A three- to five-year retrospective evaluation of all-ceramic zirconia and metal-ceramic cobalt-chromium single crowns

Abstract [sv]

Mål: Målet med denna studie var att analysera den kliniska kvaliteten och överlevnaden hos helkeramiska singelkronor i zirkonia jämfört med metall-keramiska singelkronor i kobolt-krom med en minsta uppföljningstid på tre år. I studien undersöktes även patientnöjdheten gällande de erhållna kronornas funktion och estetik. Material och metod: Fyrtionio patienter med singelkronor utförda på Odontologiska fakulteten, Malmö högskola under perioden 2008-2010 kontaktades efter en granskning av tandteknikerfakturor. Tjugofyra patienter med totalt 48 kronor (23 zirkonia samt 25 kobolt-krom) undersöktes av två tandläkarstudenter. Kronornas kliniska kvalitet bedömdes med hjälp av en modifierad version av California Dental Association (CDA) kvalitetsbedömningsmall. Patienterna ombads även att gradera sin nöjdhet gällande kronornas funktion och estetik från en skala på 1 till 10, där 1 var ”väldigt missnöjd” och 10 ”väldigt nöjd”. Resultat: Både grupper erhöll höga överlevnadsvärden (95,7% för zirkonia och 96% för kobolt-krom). Singelkronorna utförda i kobolt-krom graderades högre, både generellt och specifikt i alla underkategorier, än singelkronorna i zirkonia. Fem frakturer av ytporslinet registrerades för zirkonia, för kobolt-krom registrerades två. Patientnöjdhet gällande funktion och estetik var i genomsnitt graderad till minst 9,0 för både zirkonia- och kobolt-krompatienter. Slutsatser: Den kliniska kvaliten hos singelkronor var acceptabel för både zirkonia och kobolt-krom, dock med vissa undantag. Patientnöjdheten är hög jämfört med de kliniska resultaten.

Abstract [en]

Aim: The aim of the present study was to analyze the three- to five-year clinical performance and survival rates of all-ceramic zirconia single crowns compared to metal-ceramic cobalt-chromium single crowns. This study also evaluated the two patient groups overall satisfaction regarding function and aesthetics. Methods: Forty-nine patients with single crowns, received during the period 2008-2010 at the Faculty of Odontology, Malmö University, Sweden were contacted after scrutinizing dental technician bills. Twenty-four patients with a total of 48 crowns (23 zirconia and 25 cobalt-chromium) were included in the study. Evaluations were made using a modified California Dental Association (CDA) evaluation system by two dental students. The patients were asked to rate the level of satisfaction regarding function and aesthetics, using a scale from 1 to 10, where 1 was “greatly dissatisfied” and 10 were “very satisfied”. Result: Both groups received a high survival rate (96% for cobalt-chromium and 95.7% for zirconia). Cobalt-chromium crowns showed a higher overall and subcategory rating according to the CDA evaluation system. Five fractures of the veneering porcelain were recorded for zirconia and two fractures for cobalt-chromium. Patient satisfaction regarding function and aesthetics was on average rated at least 9.0 for both groups. Conclusion: The clinical performance of single crowns was acceptable for both zirconia and cobalt-chromium, although within limitations. Patient satisfaction is high compared to the clinical rating according to the CDA evaluation system.
