Syfte: Att undersöka hur patienter som blivit utsatta för sexuella övergrepp upplever omhändertagandet i tandvården. Material och metod. Tretton informanter (11 kvinnor) 19-56 år, som blivit utsatta för sexuella övergrepp som barn eller vuxna och var medvetna om att upplevelsen tagit sig uttryck hos tandläkare och/eller tandhygienist intervjuades. Informanterna uppmuntrades att med egna ord och så detaljrikt som möjligt beskriva mötet med tandvården. Intervjuerna spelades in digitalt och transkriberades ordagrant. Det insamlade materialet analyserades enligt kvalitativ innehållsanalys. Resultat: Ett mönster som beskriver informanternas upplevelser identifierades och klassificerades i följande tema och kategorier. Det tema som identifierades var Omhändertagandet i tandvården av sexuellt utnyttjade patienter vilar på en instabil grund och kräver förutsägbarhet och fördelning av makt. Vidare identifierades två kategorier, där det första var Tandläkarens ”ansvar” med subkategorierna Det motsägelsefulla avslöjandet samt Fördelning av makt. Den andra kategorin var Patienten i ”fokus” med subkategorierna Aktiv assistans och Tolkning av signaler- de subtila uttryckens behov. Konklusion. Omhändertagandet i tandvården av patienter med en erfarenhet av att ha blivit sexuellt utnyttjade, vilar på en instabil grund som snabbt kan raseras om bemötandet brister. Baserat på förståelsen att patienten blivit utsatt för sexuella övergrepp, krävs en individanpassad patient-centrerad vård där att ställa frågan om våldsutsatthet är av speciellt intresse.

Abstract Objective: To explicate and describe the experience of dental care taking, from the perspective of sexually abused victims. Methods: Thirteen informants 19-56 years, who experienced sexual abuse and in whom the abuse experiences were remembered and expressed during a dental appointment, were interviewed. They were encouraged to tell in their own words and in as much detail as possible about encounters in dental care. The interviews were recorded digitally and transcribed verbatim. The collected material was analyzed according to Qualitative Content Analysis. Results: A pattern that describes the informant’s experiences was identified and classified in the following theme and categories. The theme that was identified was, Care taking in the dental setting of patients who have been sexually abused lies on an unstable foundation and demands predictability as well as a distribution of power. Furthermore, two categories were identified, the first was The dentists “responsibility with the subcategories, The reluctant exposure and Distribution of power. The second category identified was The patient in ”focus” with the subcategories Active assistants and Interpretation of signals- the subtle expression of needs. Conclusion: Treatment in the dental setting of patients who have been sexually abused lies on a unstable foundation that can quickly fall apart if the care taking has deficiencies. Based on an understanding that the patient has been sexually abused, an individually tailored patient centered treatment is suggested, where emphasis on asking the question about the experience of violence is of special interest.
