Abstract [sv]

Att förlora en eller flera tänder och vara hänvisad till att bära avtagbar protes kan, beroende på individ och situation, inverka på livskvaliteten. Kostnaderna för tandvård kan påverka såväl val av behandling som att en individ avstår från tandbehandling.Målen med denna avhandling var att:Mot bakgrund av tandvårdsförsäkringen studera tandvårdsnyttjande och oral hälsa, med speciellt fokus på orala proteser, samt oral hälsorelaterad livskvalitet i en svensk vuxen population.Beskriva dentala förhållanden – med fokus på protetiska variabler – i relation till sociala förhållanden och utvärdera om det fanns indikationer på förändringar i socioekonomisk påverkan på dentala förhållanden, sedan införandet av tandvårdsförsäkringen.Studera om olika attityder till tandvård är associerade med sociala såväl som dentala förhållanden och undersöka tandvårdsnyttjande i relation till dentala och sociala förhållanden, liksom i relation till attityder gentemot tandvårdskostnader och upplevt behov av tandvård.Utforska dimensionaliteten av variabler, ämnade att mäta oral hälsorela-terad livskvalitet, baserade på Lockers modell, i svensk kontext, samt att diskutera likheter mellan de funna dimensionerna och två utvalda befintliga instrument Oral Health Impact Profile (OHIP) och Oral Impact on Daily Performances (OIDP).Undersöka huruvida oral hälsorelaterad livskvalitet i en svensk vuxen population kunde förklaras av (a) sociala attribut, (b) individuella attribut, (c) dentala förhållanden med speciellt fokus på protetikens roll samt (d) attityder till tandvård.Enkätformulär skickades 1998 till ett slumpmässigt urval av 1974 per-soner i åldern 50-75 år, bosatta i Skåne. Svarsfrekvensen blev 66%.Resultaten visade att:Det fanns associationer mellan sämre social situation – i form av hög ålder och lågutbildad – och försämrade dentala förhållanden – i form av få kvarvarande tänder eller tandlös.Socioekonomiska skillnader avseende dentala förhållanden tycktes vara oförändrade jämfört med tidigare studier.Låg utbildning, och försämrade dentala förhållanden associerade med uppfattningen att kostnaden för tandvård är viktig och med att ha upplevt ett tandvårdsbehov utan möjlighet att tillgodose det på grund av kostnadsbarriär.Tandvårdsnyttjande var relaterat till kön, dentala förhållanden och attityder till tandvårdskostnader. Oberoende av varandra fanns där en högre sannolikhet för lågt tandvårdsnyttjande (mer sällan än en gång per år) för män, för personer med få kvarvarande tänder eller tandlösa, för de med avtagbar protes, för de som upplevde ett tandvårdsbehov utan möjlighet att tillgodose det på grund av en kostnadsbarriär samt för de som ansåg att kostnaden hade inverkat på deras tandvårdsnyttjande.Det fanns en tendens till högre sannolikhet för lågt tandvårdsnyttjande för de lågutbildade.Tre faktorer förklarade dimensionaliteten av 22 variabler för mätning av oral hälsorelaterad livskvalitet i den aktuella populationen: (1) Fysisk och social förmåga (den dominerande faktorn), (2) Psykologisk komfort och förmåga samt (3) Funktionell begränsning och fysisk smärta. Di-mensionerna var liknande de i OHIP och OIDP.Generell hälsa i förhållande till jämnåriga, tandantal och variabeln upplevt tandvårdsbehov med kostnadsbarriär, hade den starkaste inverkan på dagliga aktiviteter, på den psykologiska dimensionen och på oral funktion. Där fanns interaktionseffekt mellan tandantal och typ av protetisk ersättning.Typ av protetisk ersättning uttryckt i fast eller avtagbar protes, var mindre viktig för de med få kvarvarande tänder eller tandlös än för övri-ga, för att förklara oral hälsorelaterad livskvalitet.Sammanfattningsvis konstaterades social ojämlikhet avseende dental situation. Lågutbildade och äldre upplevde sämre tandhälsa än övriga. Målet med tandvårdsförsäkringen var vid dess införande att möjliggöra tandvård på lika villkor. Jämfört med tidigare studier kunde dock inte ses några indikationer på förändringar i socioekonomiskt inflytande se-dan försäkringens införande. Där fanns sociodentala skillnader i attityder till tandvårdskostnader vilket även avspeglades i lågt tandvårdsnyttjande. Lågt tandvårdsnyttjande kunde också relateras till lågt tandantal, protesbärande, manligt kön och låg utbildning.Upplevelsen av livskvalitet och oral hälsorelaterad livskvalitet är individuell och multidimensionell. Tre dimensioner av oral hälsorelaterad livskvalitet utkristalliserades i populationen. Fysisk och social förmåga rymdes inom samma dimension som också var den dominerande. Den oralt hälsorelaterade livskvaliteten beskrevs i tre dimensioner: Dagliga aktiviteter, Psykologisk dimension och Oral funktion. En av de mest framträdande förklaringarna till oral hälsorelaterad livskvalitet var antal kvarvarande tänder. Det fanns en teori att förekomst av avtagbar protes skulle vara av stor betydelse för försämrad livskvalitet. Det fanns dock interaktionseffekter mellan tandantal och protesförekomst vilket tolkades som att upplevelsen av tandförlust var mer negativ än förekomst av avtagbar protes vid värdering av oral hälsorelaterad livskvalitet. Det var också mindre viktigt för kategorin med få kvarvarande tänder eller tandlösa om den protetiska konstruktionen var fastsittande eller avtagbar. Avhandlingen visar som helhet att sociala och dentala förhållanden liksom tandvårdskostnad spelar stor roll för såväl vårdutnyttjande som livskvalitet. Oral protetik har en betydelsefull roll, där typ av ersättning interagerar med tandförlusten i sin effekt på livskvaliteten.

Abstract [en]

The main aim of this thesis was to study the impact of oral health and oral prostheses on oral health-related quality of life (OHRQOL) in an adult Swedish population. Additional aims were to study social inequali-ties in oral health, attitudes towards the cost for dental care and dental care utilization. The study base was 1294 responses to a questionnaire from a random sample of 1974 persons aged 50-75 years, all of whom were resident in the County of Skåne, Sweden. There was an association between impaired dental conditions and poor social conditions. Low dental care utilization covaried with impaired dental conditions and with stating a perceived need to obtain dental care but with no possibility to obtain it because of a cost barrier. In factor analysis, three factors captured 22 variables that aimed to measure OHRQOL. The constituent variables were summed into three index variables interpreted as oral health impact on everyday activities, on a psychological dimension and on oral function. The three variables were set as dependent variables in regression models with the independent variables social attributes, individual attributes, dentures, number of teeth and dental care attitudes. The models were run in three steps taking into account the interaction between the type of denture and the number of remaining teeth. The number of remaining teeth was more important than the type of denture when explaining OHRQOL. The type of replacement, in terms of fixed or removable denture, was less important for those with few or no remaining teeth, than for all others. OHRQOL was also explained by general health in relation to age peers as well as by varying attitudes towards dental care costs. Statistically significant interactions were ob-served between the number of remaining teeth and the type of denture when explaining OHRQOL. As a whole the thesis shows that social and dental conditions and cost for dental care play a great role for dental care utilization as well as for OHRQOL. Prosthodontics has an important role, where type of replacement interacts with tooth loss in its effect on QOL.

Abstract [en]

This study aimed to: describe dental conditions--focusing on prosthodontic variables--in relation to social conditions in the late 1990s in an adult population of Southern Sweden, evaluate if a change could be traced in the pattern of socioeconomic influences on dental conditions, and study if various attitudes toward dental care were associated with social as well as dental conditions. The study was based on questionnaire responses. Significant differences in dental conditions and denture prevalence were found for age and education. To a majority of the sample it was very important to have own teeth and/or fixed restorations and the opportunity to attend regular dental care. The cost for dental care was very important for 52% of the sample especially for men, those with low education, and those wearing removable denture. Need for dental care that could not be provided for because of the costs was experienced by 9%. Eighteen percent stated that they once or more had refrained from dental care because of the cost. Those with removable dentures and low education dominated. Besides socioeconomic differences in dental conditions, there were sociodental differences in attitudes concerning the importance of costs, self-estimated needs, and cost-barriers for dental care.

This study investigates dental care utilization in an adult population in Southern Sweden in relation to dental and social conditions, attitudes to costs, and perceived need to obtain dental services. The study was based on responses to a questionnaire sent in 1998 to a random sample, 1974 persons, aged 56-75 years. The response rate was 66%. A significantly higher probability of dental care utilization less than once a year was found for men, for those with few remaining teeth, and for those with removable dentures. A higher probability of dental care utilization less than once a year was found for those who stated perceived need to obtain dental care with no possibility because of the cost and for those who stated that the cost had influenced their attendance for dental care. The results showed that there were differences for sex and dental conditions in dental care utilization and that dental care utilization was related to attitudes towards costs of dental care.

The concept of oral health-related quality of life (OHRQOL) is investigated in this study. The aim was to explore the dimensionality of variables measuring OHRQOL in an adult Swedish population and to discuss the findings in relation to existing indices. The study was based on responses to a 1998 questionnaire sent to a random sample of 1974 persons aged between 50 and 75 years. There were 22 variables based on questions concerning oral situation and the impact on 7 theoretically different dimensions of QOL. The majority were satisfied with their oral health situation. During the previous 12 months, 16% of the population had experienced problems with their mouth or teeth on at least one occasion per mouth. Principal components analysis was used to analyze the dimensionality of the variables. Three factors accounted for 59% of the variance: (1) Physical and social disability, (2) psychological discomfort and disability, and (3) functional limitation and physical pain. The perception of OHRQOL is multidimensional, but the dimensions are not equally important. The dimensions of OHRQOL found in the present study are similar to those of existing instruments.

PURPOSE: The purpose of the study was to investigate whether oral health-related quality of life (OHRQOL) in an adult Swedish population could be explained by social attributes; individual attributes; dental status, with a special focus on the role of prosthodontics; and dental care attitudes. MATERIALS AND METHODS: The study was based on responses to a questionnaire sent in 1998 to a random sample of 1,974 persons aged 50 to 75 years (66% response rate). Three factors representing various aspects of OHRQOL were set as dependent variables in multiple-regression models: oral health impact on everyday activities, oral health impact on the psychologic dimension, and oral health impact on oral function. Independent variables in the models were social attributes, individual attributes, number of teeth, denture (ie, type of denture, if present), and dental care attitudes. RESULTS: General health in relation to age peers had the strongest association with all 3 dependent variables, followed by number of teeth and need care--cost barrier. When number of teeth was excluded, removable denture was found to covary with the dependent variables in each of the 3 regression models. CONCLUSION: The number of remaining teeth is more important than the type of denture in explaining OHRQOL. It is less important that a denture is fixed for those with few remaining teeth, in contrast to all others. Explanations are also found in general health and various aspects of dental care costs.
